Creating an eForm
To create a new eForm, navigate to the eForm administration module by clicking on "Administration" from your schedule page, then "Forms/eForms", and finally "Manage eForms".
In the Manage Eforms section, click on the “Create” button.
To start creating an eForm, give it a name and then click “Next”.
You can upload a background image for your eform by dragging it onto the whitespace. The background can be either a PDF or a picture file (png, Jpeg).
Adding Text Fields to Your eForm
Now that you have your background set, you can start to add text boxes to all the fields that require them. To get started, click on the "A - Add Textfield" icon, then move to the appropriate area of your form and click again to place the text field.
When you are done adding text fields, please "esc" on your keyboard to turn off the text-field item selector.
Next, you can start assigning database tags or measurement tags to the text fields. You can do this by searching or choosing from the list.
A measurement tag can be a value (e.g. blood glucose, blood group, oxygen saturation, etc) or an observation date.
You can also add check boxes by clicking the "Add Checkbox" icon.
Groups of check boxes can be selected and easily aligned vertically or horizontally.
Creating a Gender Checkbox
For eForm checkboxes used to identify the patient's gender, click on the checkbox to select it, and then in the properties field, click beside "Gender checkbox". You can now select the appropriate gender to associate with the checkbox.
Adding a Signature field
You can also add a signature box to the signature field.
You will need to type in your PIN in order to insert your signature. The signature will have been previously saved in your account.
Once you have completed the form, click “Save & Close”.
Adding an eForm to a patient's chart
To use your eForm, click on the eForms’ + from the patient’s chart.
The eForms tab will open up. Choose the eForm you'd like to use from the list, or search for it in the search field.
The eForm will open with all of the text fields prefilled (using data from the patient’s file).
To save the eForm to the patient's file, click the "Save and Close" button.
