Electronic forms or eForms replaces the need for paper or PDF forms, saving you time and resources. eForms allows you to auto-populate patient information, add error-free calculations and directly fax the final result - no scanning involved!
To complete an eForm for a patient, select the ‘+’ button beside the eForm section in the left panel of the patient's eChart. Your eForm library will pop-up where you can search and select an eForm to fill out.
On the left side of the pop-up display is your eForm groups and on the right, the entire eForm library with a search bar for quick selection.
Once selected, the eForm will open up in a new window, pre-populating the specified text fields with corresponding data from the patient’s database.
Fill out your eform using the interactive fields and then sign-off. Once finished, you can fax, download, print or save your eForm right from the same window.
💡 You can attach documents from the patients eChart to include in your eForm by selecting the 'Add Attachments' button💡

Trying to fill an e-form - it is using extremly miniscule and tiny font, no option to increase font size. Frustrating experience
