If a document has been linked to the wrong patient, you will need to download the document, upload it to the correct patient’s chart, and then remove it from the incorrect chart.
Steps to Reassign a Document
1. Open the Incorrect Patient Chart
Navigate to the patient chart where the document was originally uploaded.
Locate the document in the patient’s 'Document' section.
2. Download the Document
Open the document.
Download the document to your computer.
3. Upload the Document to the Correct Patient
Open the correct patient's chart.
Click the '+' button next to 'Documents'.
Upload the document you downloaded.
4. Remove the Document from the Incorrect Patient
Return to the original patient’s chart.
Click on the 'Document' title to view all documents.
Locate and delete the incorrect document.
