If a patient comes to your clinic and their rostered physician isn't available, Avaros' link feature lets you quickly connect to their existing chart at another clinic in your FHO group. So you don't have to start from a blank chart. This article walks you through how to link and unlink patient charts, and what to expect once a chart is linked.
How to Link a Patient Chart
- Open the patient's eChart.
- Click the menu in the top left corner of the chart.
- From the drop-down menu, select Link.
- A new window will show whether the patient's demographic information matches any other patient chart at another clinic in your FHO group.
Match:
No Match: - If a match is found (for example, at another clinic in your group), select the link icon to connect the two charts.
- That's it, you'll now have quick access to view the patient's medical records from the linked clinic.
What You'll See on a Linked Chart
Once a chart is linked, you'll notice:

A linked-data icon on any entries originating from the other clinic. Hover over it to see which clinic the entry came from.
- Access to the patient's full clinical history from the linked clinic — encounter notes, cumulative patient profile, previous labs, and more.
This gives you a much clearer picture of the patient's health background before you even get to their chief complaint for the visit.
You can open documents from linked clinics directly (Labs, imaging, and reports). However, eForms and Consultations must be requested from the clinic where they were originally created.
Sending a Tickler After a Linked Visit
After you complete your encounter note for a linked patient visit, you'll be prompted to send a tickler to the patient's most responsible provider at the other clinic.
You can enter a message to the MRP, then click Send Tickler to notify them directly in their EMR.
This lets the patient's regular provider know their patient was seen at another location.
How to Unlink a Patient Chart
If you ever need to unlink a patient chart:
- Go back into the menu in the top left corner of the eChart.
- Select Link.
- Choose the unlink icon to disconnect the charts.
